Job Description

Education:                  High School Diploma or GED Equivalent.
Experience:            Experience in the field of physical and/or mental health is a plus, however, all training will be provided by the State Health Department for the right candidate.  

Skills & Personal Characteristics:              

  • A genuine concern for homeless, disable, and or/ unsheltered persons.
  • Empathy for and understanding of problems, limitations, and needs of the unsheltered.
  • An ability to effectively communicate and network with other agencies and the public.
  • An ability to analyze socio-economic conditions, and identify unmet needs of those served on both micro- and macro- bases.
  • A willingness and ability to work cooperatively with co-workers and supervisors.
  • An awareness of various assistance programs and entitlements available to the homeless and disabled populations.
  • Flexibility and patience.
  • Strong organizational and multi-tasking skills.
  • Valid driver’s license with own personal transportation.

Duties & Responsibilities:                                  

  1. Provide information to community members, individually and/or groups, about health inequities and/or health disparities through meetings, seminars, health fairs, and advertisements.
  2. Refer clients to a broad range of appropriate resources and agencies and perform appropriate follow-ups to ensure the effectiveness of the referrals.
  3. Conduct awareness messages or campaigns to provide education on the organization’s three-tiered approach to addressing poverty and marginalized populations:
  4. Emergency Assistance – Serve as a resource to address housing rehabilitation, food insecurity, and utility assistance.
  5. Intensive Case Management – Provide bi-directional referrals to support clients for wrap-around services for a healthier life outcome.
  6. Peer Support – Serve as a mentor for clients that transition to being self-sufficient.
  7. Impart concepts of health promotion and disease prevention, acting public health advocate. (For example, health screenings, health fairs, etc.)
  8. Conduct key informant interviews with various community leaders to gain their perspective of the health care system and local needs.
  9. Translate literal and medical languages that include medical jargon language.
  10. Set up partnerships with Local Healthcare providers.
  11. Maintain an outreach log to document groups served, activities provided, and materials distributed along with evaluation tools and checklists for grant requirements.
  12. Attend all program meetings, conferences, training, and education sessions on an ongoing basis as identified through the organization and the MSDH OCHWs
  13. Conduct health programs that focus on social determinants of health and chronic illnesses.
  14. Make public presentations to community groups to provide education about the organization and the community-clinic linkage provided to local health care systems and social support, resources and services and their importance to the community
  15. Speak to mission groups/campers as needed.
  16. Other tasks as assigned by the Director of Client Services.
